My name is Tom Sheen, and I oversee all repatriation services at
Alliance Pioneer Group (APG). I am a registered Paramedic with over 20 years of experience across emergency care, patient transport and international medical repatriation. During my career, I have completed more than 500 dedicated air ambulance missions and managed many hundreds more. Supporting people at vulnerable moments, and helping them get home safely, is at the heart of what we do.
One common misconception among expats is that repatriation is only possible through travel insurance. While insurance can certainly help, many long-term residents, retirees and property owners in Spain are either uninsured or find that their policies do not cover extended stays, pre-existing conditions or certain medical events. In these situations, knowing where to turn can make all the difference.

There is no "one-size-fits-all" solution when it comes to repatriation. The most appropriate option depends on medical needs, mobility, urgency and distance. At APG, we arrange a range of repatriation services, including:

- Non-medical escorted road transport
For patients who are seriously unwell or unable to travel independently, a dedicated air ambulance may be required. These aircraft are staffed by highly experienced medical teams, including doctors, nurses or paramedics, tailored to the patient's needs. Although APG does not operate its own aircraft, we work closely with trusted international aviation partners to charter the most suitable aircraft for each mission.
APG can manage every aspect of this transfer including UK hospital admission.
For those who are more stable, a commercial flight with a medical escort can be an effective and less costly option. A qualified clinician accompanies the patient throughout the journey, with APG arranging medical clearance, airport assistance and road transport at both ends to ensure a smooth and supported transfer.
In many cases, particularly within mainland Europe, repatriation by road ambulance is also possible. APG operates modern, fully equipped ambulances with dedicated medical teams and works with a reliable network of European partners to deliver safe, comfortable transfers back to the UK or elsewhere in Europe.

For individuals without acute medical needs who simply require assistance during the journey, non-medical escorted transport by road can be a practical and comfortable alternative, especially for those who prefer not to fly.
